Overview

The Children's Community Nursing Team provide nursing care to any child, that lives within South Tyneside and has a nursing need.

We care for children aged 0 to 16 years and up to 19 years if the child is still in full time education or has special needs.

Our aim is to support early discharge from hospital and to prevent discharge wherever possible by providing care in the child's own home


Support available

The team contributes to:

  • training and assessment of need
  • medical equipment
  • delivery of care
  • work alongside relevant health professionals
  • written plans

The team also attend South Tyneside and Sunderland's special schools to deliver lunch time medications during term time.

The team are happy to deliver care and participate in meetings for children, who attend the special schools and are on the Community Children's Nursing teams' caseload.


Who can access the service

Children who:

  • are aged 0 to 16 years or 18 if in full time education and have special needs or circumstances
  • live within the Borough of South Tyneside and have a specific medical or nursing need (not already being supplied by universal services).

The eligibility is not disease or diagnosis specific but generic and applicable.


How to access the service

Referrals can be made by:

  • hospital nurses and doctors in any department
  • Health Visitors
  • GP's
  • Practice Nurses
  • School Nurses

The team work 364 days a year, Monday to Sunday, 8am to 6pm.
