Overview
The Children's Community Nursing Team work with children who have specific nursing or medical needs.
They provide care to children in their own home, where it is safe to do so.
Support available
The team helps with:
- training and assessment of need
- provision of medical equipment
- delivery of care and / or work with health professionals, to contribute towards written plans
The team respond to children needing urgent care, assess their clinical status and help provide treatments such as:
- intravenous antibiotic therapy
- wound care
- enteral feeding management
The team also attend South Tyneside and Sunderland's special schools to deliver lunch time medications during term time.
The team can help to deliver care and take part in meetings for children who attend the special schools and are on the Community Children's Nursing teams' caseload.
Who can access the service
Children who:
- are aged 0 to 16 years or 18 years, if in full time education and have special needs or circumstances
- live in South Tyneside and have a specfic medical or nursing need, that is not already supplied by universal services
How to access the service
Referrals can be made from South Tyneside and Sunderland NHS Foundation Trusts:
- family services
- wards
- accident and emergency
- urgent care
- special care baby unit
Referrals can also be made by:
- other professionals
- hospitals
- GPs
- community care
Once the referral has been made, the child will be given a named nurse / caseload holder, who will make contact with the family to make an appointment to address their needs / requirements.
