Preparing for changes to your education, health and care services when you turn 18.

Moving into adulthood is a big change for young people, especially when they have SEND.

There are a lot of decisions that need to be made that impact you and your family.

The support that is available in education, health and care services will change when you reach 18, so it is important that you prepare for this.

Young people with Special Educational Needs and Disabilities (SEND) tell us that they want to live a life where they can have the same experiences as other young people without SEND.

They want to have choices and control of the decisions that are made to make sure they can:

  • Succeed in education training and employment
  • Live as independently as possible
  • Have good health
  • Be active in the community
